WHAT IS FISTULA-IN-ANO?
Fistula in ano is an abnormal tract or tunnel created from inside of the anus to outside skin. Generally, it forms in the upper part of the anus where anal glands are present. The infection of these anal glands creates a collection of puss pocket, called abscess. This pocket of puss or abscess that needs to drain, creates a fistula tract.

Fistulas are classified on the basis of their location w.r.t sphincter
muscles. Sphincter muscles play a crucial rule in bowel controls. So

wrong surgery can damage your sphincter muscle.
I. INTERSPHINCTERIC FISTULA
Most common form of fistula which passes through internal
sphincter muscle & develops between internal and external
sphincter muscle.
II. TRANSSPHINCTERIC FISTULA
It passes through both layers of sphincter muscle.
III. SUPRASPHINCETRIC FISTULA
It passes through internal sphincter and goes around
external sphincter.
IV. EXTRASPHINCTERIC FISTULA
This is a rare kind of fistula which goes around both
sphincter muscles. This generally originates from rectum.
V. SUPERFICIAL FISTULA
This originates from lower part of anal canal, below anal
glands & by passes the muscles.
2. FISTULA CLASSIFICATION AS PER THE SEVERITY
(i) SIMPLE FISTULA
Single tract fistula without any branching and which
do not occupy space in sphincter muscles are known
as simple fistulas and considered as easy to treat.
(ii) COMPLEX FISTULA
Fistulas which occupy more space on sphincter
muscle and fistulas which communicates with
sensitive part of human body are known as complex
fistula. These are hard to treat fistulas which requires
multiple surgeries and still no guarantee of cure.
(1) MULTIPLE TRACT FISTULA
Patient can observe more than one opening of fistula
outside anus. There can be even 3 to 4 different
openings in very severe cases, these external
openings even linked with each other internally and
making a complex net of fistula tracts. These fistulas
are real challenge for surgeons and are almost having
very rare chances of healing.
(2) RECTOVAGINAL FISTULA –
Border between rectum and vagina is very narrow &
highly prone for infections. Fistula which
communicates from rectum to vagina is called rectovaginal fistula. In this patient observes pain,

This is one of the most complicated forms of fistula
where surgery is highly complicated and unsafe.
(3) RECTOPENILE FISTULA
